Please bring the following items with you to your appointment

  • Your insurance card or other health coverage information. Some insurance carriers require you to have a referral or primary care physician authorization before treatment is given. Please check with your carrier if you are in doubt.

  • X-rays or MRI's obtained within twelve or eighteen months (if available).  When possible, please bring the actual films or CD for us to review.

  • Clinic notes or operative reports from your referring physician that relate to your condition.

  • Wear loose or baggy clothing.

  • A list of medications

We accept a variety of insurance plans, as well as Worker's Compensation and NO Fault. Out of Network Benefits are welcome and we will work with you to arrange a consultation. Please give our office a call to discuss further.

Medical Records
If in the future you need to request certain medical records from our office, please be aware that the request could take ten days to two weeks before you receive the information. We will make every effort to fulfill all medical records requests as quickly as possible.

Billing Information
We accept Visa, Mastercard, Cash and Check. Payment is due when services rendered.

If you require surgery for your problem, our physicians will thoroughly explain the operative procedure, the recovery process and the type of rehabilitation program you will need. Our staff will pre-certify your procedure and discuss any financial implications with you.


Our surgeons operate out of Englewood Hospital and Medical Center, Holy Name Medical Center, Teaneck Surgical Center, and Hackensack North at Pascack Valley.

After Hours Process
We always have a physician on call in case of after hour emergencies. If you need to contact a physician after hours, call the regular office number and our answering service will notify the physician on call to contact you.

Routine prescription refills will be filled between the hours of 9:00 AM and 5:00 PM Monday through Thursday and between 9:00 AM and 3:00 PM Friday only.


Except in the case of emergencies, it is our policy to see only patients with an appointment. We fill our schedule well in advance with, in some cases, people who have waited weeks to make an appointment. We understand that it is tempting to bring someone with you in hopes that they can be seen as well during your appointment time, but in fairness to those who have waited, we are not able to see anyone without a scheduled appointment. We appreciate your cooperation in this matter.
